
ACT Team (Assertative Community Treatment Team)
The ACT Team serves individuals living with severe and persistent mental illness
(SPMI). The team works with individuals to maintain them in their respective communities.
The ACT Team offers clinical counseling, medication management, job skill development,
psycho-educational groups, and various other services to those living with SPMI.
Residential Services for Adults
LFS adult group homes provide services to clients who are coping with various degrees
of autism, developmental disability, mental illness, and symptoms resulting from
traumatic brain injury. Self-sufficiency is taught and encouraged in these homes,
though many clients will live out their lives with LFS.
Transitional Apartments
LFS transitional apartments offer individuals living with severe and persistent
mental illness (SPMI) the opportunity to live on their own in the community. LFS
works with residents to develop effective independent living skills during a transition
period of about 12-24 months. Residents seek employment, follow treatment regimes,
and learn how to live on their own.
