Individuals diagnosed with a severe and persistent mental illness (SPMI) often have a difficult time functioning in community living. The
Assertive Community Treatment Team (ACT Team) Program gives individuals with SPMI access to the level of services they need to maximize their quality of life while maintaining independence in their respective communities.
The ACT Team offers clinical counseling, medication management, job skill development, psycho-educational groups, and various other services to those living with SPMI. Approximately 90% of services are provided in clients’ homes, places of employment and places where they spend leisure time.
The goals of the ACT Team are:

To lessen/eliminate symptoms of mental illness and to minimize or prevent recurrent acute episodes of the illness

To reduce the utilization of inpatient treatment facilities

To reduce frequency of after-hours crises

To reduce involvement with the criminal justice system

To improve the quality and stability of the client’s housing

To meet basic needs and enhance the client’s quality of life

To improve functioning in social and employment settings

To ease the family’s burden of providing care
